To assist you in preparing for our initial conference, we have compiled a list of information that will be needed now or in the near future. This list will guide you in gathering the necessary details when planning for the immediate future:
- Full name and address of your loved one
- Date of birth
- City/Country of birth
- Parent's names, including mother's maiden name and birthplaces
- Family names (spouse, children, grandchildren, siblings, etc.)
- Social Security Number
- Military discharge paper (DD 214) if your loved one is a veteran
- Years of education
- Occupation and employer
- Hobbies, interests, and personal items
- Affiliations (church, clubs, organizations, etc.)
- Clothing and jewelry preferences
- Cemetery deed
- 25 to 35 photographs representing family, friends, and special memories
- Number of certified copies of the death certificate required
In the meantime, we encourage you to use our Family Guide, which is a valuable tool designed to help you share the story of your loved one's life. This guide includes sections where you can document their likes, hobbies, and interests. The more information you provide, the more personal and meaningful your experience will become.
